Eligibility Requirements

If you meet the minimum "pre-screening eligibility requirements" required by federal and state laws, you may qualify for treatment with us.

Pre-screening eligibility requirements:

• You are 18 years of age or older.
• Opiates are your primary drug.
• You have been addicted or dependent on opiates for 1 year or longer (minimum 1 year) and you can prove this by one of the following methods:
  1. previous treatment with us
  2. letter from a physician or previous treatment center
  3. observable physical dependence; i.e. classic withdrawal symptoms, track marks, etc.
     
     Even if you do not meet these requirements you may still qualify if you are:
     
     
  4. currently pregnant
  5. recently discharged from a chronic care facility
  6. released from prison within the past 6 months

Intake

• Intake time is approximately 2 hours.

• We accept cash or credit cards only and do not provide billing services for insurance or medical coupons at this time.

• If you owe any back fees from previous treatment with us they must be paid at time of your intake, unless you have made other arrangements approved by the Clinic Director.

You will need to bring the following items with you to your intake appointment:

• Cash, money order, bank cards or cashier checks only to pay for treatment.
  Sorry, no personal checks
• Valid current photo ID (drivers license, military, state ID card, etc.)
• Four 2x2 inch facial photos

*Treatment fees:

• Methadone - $310 if paying on a monthly basis; $10.00 – 12.50 per day if paying on a daily basis.
• Buprenorphine - $400 if paying on a monthly basis; $15.00 per day if paying on a daily basis.
• Some locations charge an admissions fee.
